

Basic rules
Bringing direct primary care and pain management to an area of NC/TN/VA that has significant need.
Here is what we know so far:

We will not be filing insurance, this will be a membership based business.

Establishing care does not guarantee we will manage a particular patient's pain.

There will be a membership fee that will be charged monthly.

In addition, there will also be set office visit fees so there is clear up front pricing. (see below)

If we cannot manage your pain at our office, you will know within three visits (so long as records are provided as requested}.

In the event that you are deemed not appropriate for chronic traditional opioids, you may be offered MAT in for form of Suboxone or Subutex.

When we first open, it will be only on one or two Saturdays per month. Because of this limited scheduled, we will recruit no more than 50-60 patients at first.

You MUST live in one of the counties above and the address must be on your state approved ID.

Regardless, no patient visit is going to occur before August 2026. I will update this if we make progress faster than anticipated.

EXPECT everything you don't like about about a pain clinic, Random pill counts, Urine toxicology and the need to bring your controlled meds to every appointment. In order to protect our ability to help you, this will be a tightly run "ship".
Current Status
If interest continues to grow and as the weather warms, it is highly likely we may be able to start seeing patients BEFORE August - no promises yet, but you know that just means I won't promise until I can keep it.
To do that 6 main things must be in place:

1) a physical location to see you, I am working on a temporary location

2) an EHR - set up with online patient portal!

3) Malpractice insurance and collaboration agreement. I already have a physician who will help me with this (my current business partner).

4) New patients packs - accomplished by filling out new patient questionnaire when you sign in for first visit.

5) Urine drug screens will be collected at your patient visits (the face to face ones obviously).

6) The low cost telehealth visit for intake is only for the first 60 patients, after this, most will pay the flat visit fee at their first visit.

Fee Schedule

Here are the fees as they currently are being billed.

1) Membership $25/month - must be continual member to have services.

2) Visit rate: $120 per month (currently enrolled patients will be on reduced fee for rest of year)

3) Update visits - for when we rarely need to meet between visits (i.e. you have a minor illness and need a prescription) - $60

4) Joint injections $80 per joint, including the meds.

5) Urine drug screens - the vendor charges $99 cash or whatever your insurance passes to you. The urine drug screen DOES detect Kratom.

6) Shaving benign lesions on feet (think plantar warts) $100.
βIn the future we will have discounted lab on site, but this will not be available until permanent location is set.
