This first picture is what you will see in upper right corner of homepage of fourseasonspcpm.com. You may now register as an OFFICIAL patient, not just expressing interest.

When you click on login: you will go to our patient portal page - I know!!! So exciting!!

You will see this, click register as a new patient.

See the button at the bottom of the screenshot? That is where you click.

Once you are here, fill out all prompts but espescially the ones that have RED *. There is more than one page, so make sure you keep hitting next until you get the message that says your information has been submitted.

Now - once I have your information, THEN I will send information that will let you both get set up on membership ($25 per month) and also submit for a $50 telehealth visit that will be done in May sometime getting us ready for our first clinic day.

Since 1/2 of you did not click last email, I will say again. First clinic day will be June 6th and will fill up FIRST COME FIRST SERVE, so get started on this process as soon as possible.
